I think that parent and teacher relationships are a very important component in education. As I have stated before, I believe in holistic learning. As such, the students whole-living experience must be incorporated.
Here is an interview a sample interview that I created for a parent with exceptional needs. As with all sections, I hope to add more as I progress!
Module 2- Cumulative Activity:
Create a Parent/Teacher Questionnaire
for Exceptional Student Assessment
I choose to make my parent questionnaire generic. I wanted to create a template that could be adapted for future teaching experiences.
I have divided the questionnaire into categories to help me organize my questioning goals:
My interview goals are as follows:
Was there early child complications for ________________NAME OF STUDENT (birth or early trauma?)
Has ________________ ever been tested for any unique qualities?
Is there any exceptional differences you noticed with ________ during his/her younger years?
PART 2: General living conditions:
What is the student’s current living situation: who lives in the household, how many children do you have?
Do you expect the current living conditions will change in any way in the near future?
PART 3: Parental background and how this relates to the student:
What are your interests and hobbies?
What was your experience like in this grade?
What subjects did you enjoy while you were learning?
What subjects did you hate?
PART 4: Parental perception of exceptionalities and goals they have for their child:
What do you see as ___________________greatest strengths or skills?
What does _________ do well at home? What are the interests of him/her?
Does ___________________ have responsibilities at home?
How does ____________________ prefer to spend his/her spare time?
Does _____________ have a peer group outside of school?
Are there any issues at home that you notice?
What seems to work and what doesn’t work with _____________ at home?

- Assess the general living conditions at home.
- Understand the parents own background and how this relates to the student.
- Understand parents own perception of exceptionalities and goals they have for their child, and identify the parent’s educational expectations for their child.
- Understand bridges that need to be created to facilitate student learning.
